In order for NYCOMEC and your hospital to comply with the American Osteopathic Association’s accreditation and OPTI requirements, the following information needs to be completed by EACH INTERN/RESIDENT, prior to their beginning or continuing training in July 2008:

· Training Institutions will ensure that trainees are provided with an annual contract which outlines the terms and conditions of their appointment. A sample Intern/Resident Contract for hospitals can be found in the Basic Documents for OPTI and Postdoctoral Training Programs on the DO Online website, specifically, Section 2.55 and Appendix 18.

A COPY of the completed contract should be sent to NYCOMEC. The original should be held in the Intern/Resident’s file at your institution.

· NYCOMEC Intern/Resident Registration Form (Must be completed each year so that our database remains current)

· Please remember, contracts must be sent to the candidate's attention and completed at the earliest date, as outlined in the match rules, and returned to the sponsoring institution within 30 days.

Once the above information is completed, it should be forwarded to the NYCOMEC office.
